  To have a particular intention for those who are sick, recently deceased, or otherwise in need of prayers considered for inclusion in the Prayer of the Faithful during our Sunday liturgy, please use the form below or call the parish office at 847-885-7700.
  
  These requests will be read by a member of the our staff. To make sure that your request is considered for inclusion for a given Sunday, please submit no later than 5:00 PM on Thursday for the Sunday you are requesting. (If you wish for the name to be included in the bulletin, please submit no later that 5:00 PM on Monday).
  
  To request a Mass Intention, or Mass is memory of a loved one, please complete the online form, drop by, or call the parish office at 847-885-7700.
